    Abstract: An apparatus comprises a body, a shaft assembly, an end effector, and a shield member. The shaft assembly extends distally from the body. The end effector is located at a distal end of the shaft assembly. The end effector comprises an ultrasonic blade and a clamp arm. The ultrasonic blade is configured to vibrate at an ultrasonic frequency. The clamp arm is movable toward the ultrasonic blade to compress tissue against the ultrasonic blade. The shield member is selectively movable from a first position to a second position in response to movement of the clamp arm toward the ultrasonic blade. The shield member is configured cover at least a first portion of the ultrasonic blade in the first position. The shield member is configured to uncover the first portion of the ultrasonic blade in the second position.

The end effector comprises a clamp arm and an ultrasonic blade in acoustic communication with the ultrasonic transducer. The ultrasonic blade is operable to deliver ultrasonic vibrations to tissue. Pivotal movement of the trigger causes movement of the clamp arm. The trigger includes a compliant feature configured to limit the amount of force delivered to tissue by the clamp arm. The flexible feature may comprise a flexible band, living hinge, a series of living hinges, or a flexible tab.

    Abstract: A surgical instrument includes an ultrasonic waveguide extending through a body assembly. An ultrasonic blade connects to the ultrasonic waveguide. A clamp arm assembly of the surgical instrument is able to move from an opened position for receiving a tissue toward a closed position for clamping the tissue. The clamp arm assembly includes a clamp body and a clamp pad facing the ultrasonic blade. A clamp arm actuator of the surgical instrument is able to move from a first position toward a second position to direct the clamp arm assembly from the opened position toward the closed position. A modular coupling of the surgical instrument connects to the clamp pad such that at least the clamp pad can be disconnected relative to the ultrasonic blade for replacement thereof.

The selectively lockable rotation system further comprises a switch member supported for selective longitudinal movement relative to the housing and the mounting member between an unlocked position wherein the mounting member is freely rotatable relative to the housing and a locked position wherein a locking formation on a distal end of the switch member is in retaining engagement with the mounting member to prevent the mounting member and the elongate shaft from rotating relative to the housing.

    Abstract: In various embodiments, a surgical stapling instrument can include a handle, a shaft extending from the handle, wherein the shaft defines an axis, and a disposable loading unit which is assembled to the shaft in a direction which is transverse to the shaft axis. Such a connection between the disposable loading unit and the shaft can prevent, or at least inhibit, the disposable loading unit from being unintentionally displaced proximally and/or distally relative to the shaft of the surgical instrument. The surgical stapling instrument and/or disposable loading unit can further include a threaded collar and/or detent assembly configured to hold the disposable loading unit in place. In various embodiments, a disposable loading unit can include a lockout feature which can prevent, or at least inhibit, an expended disposable loading unit from being reassembled to the elongated body of the surgical instrument.

The head or blade is in acoustic communication with the ultrasonic transducer such that the ultrasonic transducer is operable to drive the ultrasonic blade to vibrate ultrasonically. The head or blade has a curved distal face. The curved distal face defines a proximally extending concave curve. The transducer and head or blade may be driven using a control logic that is configured to cause the ultrasonic transducer to generate a first vibration set followed by a second vibration set. The first vibration set is configured to generate microbubbles in a fluid. The second vibration set is configured to grow microbubbles generated by the first vibration set. The control logic may provide a pause between the first vibration set and the second vibration set.

The disposable loading unit comprises a cutting member, an anvil, and a first replaceable stapling jaw assembly. The first replaceable stapling jaw assembly comprises a first cartridge body and first staples. The disposable loading unit further comprises a second replaceable stapling jaw assembly comprising a second cartridge body and second staples. The disposable loading unit further comprises a lockout that prevents the first replaceable stapling jaw assembly from being fired after the first replaceable stapling jaw assembly has been at least partially fired. The surgical stapling instrument and the disposable loading unit are configured to deploy the second staples from the second replaceable stapling jaw assembly with which the first replaceable stapling jaw assembly is replaced.

A rotation feature may provide rotation of the blade relative to the clamp arm about the longitudinal axis of the waveguide. Alternatively, the rotation feature may provide rotation of the clamp arm relative to the blade about the longitudinal axis. The rotation feature may be driven based on pivotal positioning of the clamp arm relative to the blade about the pivot axis. The rotation feature may selectively lock and unlock the angular position of either the blade or the clamp arm about the longitudinal axis at any of a number of predetermined angular positions.

The method includes machining a surgical tool from a flat metal stock, contacting a face of a first transducer with a first face of the surgical tool, and contacting a face of a second transducer with an opposing face of the surgical tool opposite the first transducer. The first and second transducers are configured to operate in a D31 mode with respect to the longitudinal portion of the surgical tool. Upon activation, the first transducer and the second transducer are configured to induce a standing wave in the surgical tool and the induced standing wave comprises a node at a node location in the surgical tool and an antinode at an antinode location in the surgical tool.

    Abstract: Disclosed is an ultrasonic medical device that may include a surgical tool having a proximal end, an end effector, and a waveguide between them, a first transducer in mechanical communication with a first face of the surgical tool, and a second transducer in mechanical communication with an opposing face of the surgical tool, opposite the first transducer. The first transducer and the second transducer are configured to operate in a D31 mode with respect to the waveguide of the surgical tool. Another aspect comprises a method of fabricating the ultrasonic medical device, in which the surgical tool is machined from a portion of a flat metal stock so that the surgical tool has a longitudinal axis oriented at an angle with respect to a grain direction of the flat metal stock thereby optimizing an operational characteristic of the surgical tool.

    Abstract: Various embodiments are directed to electrosurgical systems for providing an electrosurgical signal to a patient. A control circuit may, for a first application period, apply the electrosurgical signal to first and second electrodes according to a first mode. In the first mode, the control circuit may limit the electrosurgical signal to a first maximum power when the impedance between the first and second electrodes exceeds a first mode threshold. The control circuit may also, for a second application period after the first application period, apply the electrosurgical signal according to a second mode. In the second mode, the control circuit may limit the electrosurgical signal to a second mode maximum power when the impedance between the first and second electrodes exceeds a second mode threshold. The second maximum power may be greater than the first maximum power.
