  • Patent number: 10978185
Abstract: Methods, systems, and computer-readable media are provided for aligning a patient who is determined to be eligible for care management services with a care manager. Patient health data is received and is used to generate a patient profile for the patient. The patient profile comprises, for example, demographic information, geographic location, clinical characteristics, patient preferences, a patient need level, a category to which the patient is assigned, and a priority level associated with the patient. A set of characteristics associated with a group of care managers is accessed where the set of characteristics include clinical expertise of the care manager, geographic location of the care manager, workload capacity of the care manager, and the like. Using the patient profile and the set of characteristics associated with the group of care managers, the patient is aligned with a care manager well-suited to help the patient achieve care management goals.

  • Publication number: 20200111557
Abstract: Systems, methods, and user interfaces provide integrated coordination of care. Care team coordination and collaboration is promoted by bringing together each of the necessary elements of a patient's plan into a single point of access. Current workflow silos that exist in the care planning space are eliminated. This allows seamless support for the many different care planning regulations across care settings and supports and involves the entire care team including the patient and personal care team.

  • Patent number: 9775569
Abstract: Systems, methods, and computer-readable media are provided for displaying on a user display device alarm-triggering events detected by the medical device based on the location of the user display device. The method comprises receiving alarm-triggering patient data from a medical device and determining the physical location of first and second user display devices. The method further comprises determining that the physical location of the first user display device is designated to have an alarm displayed, and that the physical location of the second user display device is not designated to have an alarm displayed. In response to these determinations, the content of the alarm is displayed on the first user display device.

  • Patent number: 9761035
Abstract: Methods and systems for dynamic user interfaces are provided. A user interface allows a user to receive information about a computer system's state and to make changes to state, such as with touch screen devices. Dynamic user interfaces provide advanced methods of interfacing with the computer system, receiving information, and changing computer state. Advanced methods include improved gestural controls like interrupting or fast-forwarding an animated transition. Advanced methods of receiving information from the computer system are also provided, such as real-time data updates mid-animation and meaning conveyed through motion of and/or configuration change of UI elements. Defined animation pathways in the system can have different relative velocities as a function of percentage completion regardless of the duration(s) of the animation pathways, allowing for more fluid UIs.

  • Publication number: 20130268286
Abstract: Systems and method for identifying and providing protocol variances are provided. In embodiments, the method includes referencing an ordered protocol associated with a health condition for a patient. Such an ordered protocol includes a plan for a course of health or medical treatment for the health condition of the patient. A standard protocol that corresponds with the ordered protocol is also referenced. Thereafter, the ordered protocol is compared to the standard protocol to identify at least one protocol variance that indicates a portion of the ordered protocol has been adjusted as compared to the standard protocol. Any identified protocol variances are presented, for example, to a user computing device for display to a user.
