The first end of the shank defines a first end of the first member, and a head of the first member disposed at the second end of the shank and a top of the head of the first member defines a second end of the first member. The head of the first member is frustoconical and the first member includes a longitudinal passage in the shank. A radial opening in the head of the first member is in fluid communication with the longitudinal passage.
