  • Patent number: 10194800
Abstract: The invention provides for remote patient management system comprising a computing device comprising a processor. The computing device further comprises a computer-readable storage medium containing instructions that when executed cause the processor to perform a method of calculating an assessment score. The assessment score measures the effect of a content element on a patient. The method comprising the steps of: delivering the content element to the patient, wherein the content element comprises a list of assessment parameters; generating an assessment content element using the list of assessment parameters; delivering the assessment content element to the patient; receiving a response from patient; and calculating the assessment score using the response.

  • Publication number: 20140358579
Abstract: A system and method for evaluating a discharge planning process. The system and method perform the steps of determining whether a current set of discharge criteria matches current clinical practices, generating a matched set of discharge criteria by adapting the current set of discharge criteria to reflect the current clinical practices, generating a matching quality indicator value indicating a level of matching between the current set of discharge criteria and the current clinical practices, determining whether a user adheres to the matched set of discharge criteria, generating an adherence quality indicator value indicating a level to which the user adheres to the matched set of discharge criteria, determining whether the matched set of discharge criteria satisfies a target outcome and generating a satisfaction quality indicator value indicating a level to which the matched set of discharge criteria satisfies the target outcome.

  • Publication number: 20140207489
Abstract: A method of mapping health data acquired by a measurement device (101a-c) which is useable by a plurality of users to an appropriate health record is described. The method involves receiving (201) new health data acquired by the measurement device (101a-c); identifying (206; 209) the user to which the new health data relates; and mapping (207) the health data to at least one health record for said user; wherein the step of identifying the user to which the new health data relates comprises: identifying at least one item of context data associated with said new health data; and determining (205, 208) whether said context data corresponds to a known context for at least one user. The context data is data regarding the circumstances in which the new health data was acquired and/or the relationship of the new health data to other measurement data associated with a particular user.

  • Publication number: 20130325515
Abstract: A system and method for patient discharge planning. The system and method include evaluating a patient record including patient data parameters of a patient, predicting a change in the patient record for all possible treatment options, generating a discharge recommendation based on at least one of the patient record and the predicted change in the patient record and displaying the discharge recommendation to a user.
