Abstract: An aperture forming device is provided for making openings such as tubular holes in concrete or the like during pouring thereof. The device utilizes a tube holder secured to a concrete form; a disposable tube attached to the holder and a cap member at the other or free end of the tube. Concrete is poured around the tube on the form and, after the concrete has set, the form, tube holder and tube may be pulled out of the concrete leaving the tubular aperture. A plurality of the devices can be combined in a pattern to form openings of various shapes.
