Abstract: A system and method for disease management that effectively eliminates unnecessary layers and/or intermediaries within the healthcare management system by enabling a disease management entity to operate as an extension of a physician's office, and thus, work closely with the patients and the patient's physicians to effectively empower patients towards self-care and a higher level of awareness and understanding of their disease process and treatment. The present system and method utilizes a team of highly trained healthcare professionals and technicians working together with patients and physicians, wherein remote vital sign monitoring equipment, software systems, and standing orders and treatment protocols, are utilized to effectively manage patients with chronic conditions.
