    Abstract: An anesthesia waste gas evacuation system having a waste gas collection chamber with an inlet into the chamber at the bottom and a blower at the top for blowing gas from the chamber to an outlet connected to a building ventilation system. Vents are provided to provide communication between the chamber interior and the surrounding atmosphere so that the pressure within the chamber remains substantially at atmospheric pressure avoiding the formation of a sub-ambient pressure within the chamber. The chamber is electrically grounded and is electrically connected to a standpost so as to ground the post as well.

    Abstract: A multiple ply document for receiving recording information including a top ply having a machine readable code printed thereon, a bottom ply secured by means of a line of adhesive to the top ply adjacent one edge thereof, a line of weakening in the top ply distal from the line of adhesive to permit removal of a portion of the first ply, and at least one middle ply extending to a position adjacent the line of weakening and secured to the bottom ply by means of a line of adhesive adjacent the line of weakening.

    Abstract: A multiple ply document for receiving recording information includes a top ply having a machine readable code printed thereon, one or more intermediate plies and a bottom ply secured together by means of a line of adhesive along one edge thereof. The top and intermediate plies are secured to respective overlying and underlying plies by means of staggered lines of adhesive to secure the piles at another area of the document. The top and intermediate plies also each include a line of weakening slightly below the respective line of adhesive to permit removal of a portion of the first ply and of the middle plies and leaving an upper portion of the top and intermediate plies on the document.
