    Abstract: A biopsy procedure may have ten or more high-level steps that a particular facility is able to perform, including radiology, transfer to pathology, grossing, processing and fixation, embedding, microtomy, H&E staining, coverslipping, slide preparation, and IHC. More than seven hundred individual risk points exist throughout the performance of each of these steps, each with varying likelihood of occurrence and severity of result. A software application provides a guided assessment that may be performed to assess a risk level for the facility. The assessment identifies approximately thirty observable characteristics of the facility, each of which may implicate one or more of the seven hundred risk points. Responses to the assessment may be used to produce a current risk metric for a facility, as well as identify a post-control risk metric for the facility, which may be achieved by implementing processes and controls to address the observable characteristics of the facility.
