Abstract: An information collection system includes a network of connected electronic communications devices. The devices permit design of a survey questionnaire. The survey questionnaire is communicated over the network to another device communicatively connected to the network. A response to the survey questionnaire is input to the device that receives the survey questionnaire. The response is communicated over the network to other devices of the network. A database saves the response with all other responses. Information from responses are compiled and processed. Reports are generated of the survey results. The system is particularly useful to rate vendors and products from vendors, for example, in a supply function or operation of an enterprise.
