Abstract: Embodiments relate to population health management systems and methods. To assess the health of individual patients, data about the patients may be assessed against various criteria. When the criteria is met, the patient or her physician may be notified of potential issues. Population health management may look beyond the health of an individual patient to other individuals in a common group. Embodiments may process aspects of the population health management in real or a non-real time basis.
