    Abstract: A process of evaluating the residential clinical performance of a medical resident based upon feedback from evaluators includes generating a number of questionnaires. The questionnaires contain questions related to competency area milestones for the medical resident. E-mails are sent to the medical resident with access codes for the questionnaires. The medical resident then works with an evaluator to treat a patient, the patient receiving healthcare related to a competency area milestone. The medical resident then sends the evaluator an access code for a questionnaire related to the competency area milestone. The evaluator uses the access code to access the questionnaire and answers the questions on the questionnaire. Answers are logged in a database.

A database is created containing a number of key terms related to one or more of the following: ICD-10 billing codes; medical conditions; treatment; or diagnoses. The medical professional asks the patient a question. The question is converted into text. A number of key text terms are identified from the text by determining which words from the text match key terms from the database. The medical record is updated based on the key text terms. The medical professional then reviews the medical record and repeats the prior steps as necessary to complete the medical record.

    Abstract: An ICD-10 paper coding system includes a first paper with a first table and a plurality of first cells which include clinical diagnostic terms. A second paper is attached to the first paper which has a corresponding second table with a plurality of second cells. The second cells contain terms corresponding to ICD-10 codes which relate to clinical diagnostic terms in the respective corresponding first cells. The tables have the same alignment such that when the first paper is placed over and aligned with the second paper, the first cells align with their corresponding second cells. The second paper can be carbon paper, such that when a clinical diagnostic term is marked on the first page, a reciprocal mark appears on a corresponding ICD-10 code on the second page.
