    Abstract: An apparatus comprises a sleeve, a seal, and an anchor assembly. The sleeve is flexible and non-permeable and is configured to fit in a duodenum. The seal is positioned at the proximal end of the sleeve and is configured to seal the proximal end of the sleeve against the stomach mucosa, the pylorus, or the duodenum mucosa. The anchor assembly comprises a pair of atraumatic anchor ends and a resilient member positioned between the anchor ends. At least a portion of the resilient member extends along at least a portion of the length of the sleeve. The resilient member is configured to bias the anchor ends generally toward each other, to thereby substantially anchor the sleeve in the duodenum. The sleeve is configured to prevent chyme from contacting the mucosa of the duodenum; and to prevent enzymes excreted in the duodenum from mixing with chyme in the duodenum.

    Abstract: Devices and methods for forming and securing a tissue plication are disclosed herein. In one aspect, a tissue manipulation device is described that includes a first jaw member pivotally coupled to a distal end of an elongate shaft and having an articulating portion located distal to the proximal end of the first jaw member. The device also includes a second jaw member pivotally coupled to the first jaw member such that the jaws move in a first plane, and a fastener delivery member attached to the second jaw member. The articulating portion of the first jaw member is configured to move the first and second jaw members between a straight configuration in which a longitudinal axis of the elongate shaft is contained within the first plane and an articulated configuration in which the longitudinal axis of the elongate shaft is transverse to the first plane.

More particularly, the devices and methods of the present invention can be used to create multiple tissue folds on an anterior and posterior wall of a stomach cavity to reduce the volume thereof. In one aspect, a tissue acquisition and fixation system is described that includes a staple applying assembly having first and second jaws, and a tissue acquisition member that extends substantially parallel to a plane running through the first and second jaws. The tissue acquisition member is effective to engage and position tissue between the first and second jaws, and the jaws are effective to apply at least one staple to the tissue engaged therebetween.

    Abstract: A method of plicating a stomach by accessing the exterior of a stomach of a patient and grasping tissue on a first and second location on a first side of the greater curvature of the stomach. Also forming a plication therebetween with a first staple. The method involves grasping tissue on a third and fourth location on a second side of the greater curvature of the stomach and forming a plication with a second staple. The first and second plications are near a location selected from the group consisting of a gastroesophageal junction, a pylorus, and an incisura angularis. The method further includes grasping tissue on a first and second side of a greater curvature of the stomach and forming a fold along a length of the greater curvature by attaching the first and second sides together along a first attachment line with a first plurality of staples.
