Abstract: Systems and methods are disclosed for data driven document creation and modification. The systems and methods include obtaining a first dataset having data records associated with entities, obtaining a list of entities associated with a first subset of data records in the first dataset, and obtaining configuration information, wherein the configuration information includes rules for identifying logical relationships in the data records and wherein the configuration information is specified using a vector-oriented language. The systems and methods further include extracting, for each entity in the list of entities, based on the rules, data records from the first subset of data records associated with the entity and generating a document for each entity in the list of entities using the extracted data records and the configuration information.

Abstract: Systems and methods are described for providing consumers with access to the discounted rates for health care procedures provided by dentists, primary care physicians, specialist physicians and facilities. Fee information for contracted providers is stored in a database and is accessible to consumers via an online interface. Dentists or health care providers can receive payment from a customer's pre-funded account in exchange for health/dental services provided to a customer, through the use of a health/dental care card. The card is issued to the customer by a health plan organization and is linked to a pre-funded account for that customer. The health care provider is under contract with the organization to offer a predetermined fee structure for covered services.

Abstract: Techniques are disclosed for allowing a heath care provider to receive payment from a customer's pre-funded account in exchange for health services provided to a customer, through the use of a healthcare card. The card is issued to the customer by a health plan organization and is linked to a pre-funded account for that customer. The health care provider is under contract with the organization to offer a pre-determined fee structure for covered services. Customers can obtain contracted rates for services via the card, regardless of any limitations that might apply under a health insurance policy or other program. Online, real-time methods are disclosed for the substantiation of payment claims to ensure they are within governmental guidelines for FSA/HRA/HSA coverage. Additional aspects are disclosed whereby health care providers can choose to be paid directly from a health plan organization via an electronic funds transfer into the provider's bank account.
