Abstract: A computer-implemented method of formulating a holistic treatment recommendation. The method includes receiving a first set of patient data based on responses to a first set of prompts relating to a plurality of priority concern areas associated with self-care management and determining, using a first algorithm, which of the priority concern areas are relevant to the patient. The method further includes receiving a second set of patient data based on responses to a second set of prompts relating to the one or more of the priority concern areas determined to be relevant to the patient and determining, using a second algorithm, which of the priority concern areas is most relevant to the patient. The method further includes synthesizing and analyzing the second set of patient data and the patient's electronic medical health data to determine a holistic treatment recommendation for the patient, which is then output on a display.
