A bottom surface of the tool insertion hole is located on the distal end side with respect to an end of the thread portion on the distal end side.

When an elastic body attached to the first shaft portion is squeezed between the hub and the first head portion of the positioning bolt as a result of the first male screw portion being screwed into a positioning female screw hole of the hub, the elastic body deforms to have an increased outer diameter as compared with the case where the elastic body is in its free state.
