The database is remotely accessible by one or more care givers and has software-based programming associated therewith. The database us populated with patients' clinical records and is remotely accessible to electronically retrieve at least a portion of the patient's clinical records. The clinical records are downloaded and displayed for viewing by the caregiver. Advertisements can also be displayed for viewing by the caregiver upon accessing the database.

Abstract: In a system and computer implemented method for digitally recording progress notes a content database is operable with a computing device to populate information relevant to patients and care providers. Selected items are displayed on a screen of a computing device. The computing device operates to generate information mandated by one or more certifying authorities by causing questions pertaining to the mandated information and selectable answers to the questions to be exhibited on a display of the computing device. At least a portion of the questions displayed are dependent on the answers chosen by the user during operation of the computing device. The selected answers are stored into the content database.

Abstract: A diagnostic encoding method is provided. The method includes assigning a first plurality of identifiers to an anatomical structure of the human body and assigning a second plurality of identifiers to diagnostic criterion by which caregivers evaluate patients. The method further includes, when diagnosing a patient, recording findings of the diagnosis of the patient using at least one of each of the first plurality and the second plurality of identifiers. In one embodiment, the first plurality of identifiers includes a first subset of identifiers identifying each organ in the human body (e.g., a right ear) and a second subset of identifiers identifying component parts of each of the organs in the first subset (e.g., an ear drum of the right ear). In one embodiment, the second plurality of identifiers includes a third subset of identifiers identifying symptoms relating to the first and the second identifiers.

The software is customizable to allow a user to determine at least a portion of the questions and the answers that are displayed and is operable to cause text to be input into a letter. The software is customizable to allow a user to determine at least a portion of the text that will be input into the letter upon selection of at least a portion of the answers and is operable to generate the letters in accordance with the answers chosen by a user.

Abstract: In a method for tracking medical test information a content database is established and is cooperable with software, the content database being populable with identifying information relevant to particular patients and care providers. Information relevant to tests performed or to be performed on one or more of the patients is received and associated with the patient. The information stored in the database and retrievable therefrom, includes; the type of test, who ordered a particular test, when it is ordered, when was the scheduling done for the test, who scheduled it, when was the test scheduled to be performed, when was it performed, what was the expected date of the test results, when did the results actually become available, when were the test results read, when were the test results conveyed to the patient, and when did the final disposition occur. The software being operable to sort at least a portion of said information upon request by a user.
