Abstract: A healthcare patient application is configured to interface with healthcare provider's electronic medical records (EMR) systems to send/receive to/from healthcare provider's EMR systems patient medical records using a standardized clinical document that is transmitted by the healthcare provider to a patient application or by the patient to the healthcare provider application and EMR systems. The healthcare provider may approve some or all of the standardized clinical document for integration with their EMR system. Likewise, the patient may approve some or all of the standardized clinical document for integration with the patient's electronic personal health record (E-PHR), the E-PRH being a compilation of the patient's received EMR from one or more healthcare providers. Similarly, the patient may select some of all of the E-PHR for transmission to one or more healthcare provider applications and EMR systems.
