    Abstract: Methods, systems, apparatus, and non-transitory computer readable media are disclosed for providing a common application that allows a customer to submit relevant information across various channels for obtaining insurance policy products. Various aspects include a common application that receives information entered by a customer via one or more customer channels, which may include online channels, call center channels, insurance agent channels, and/or any other suitable channels. Various aspects also include aggregating portions of the relevant customer's information while identifying the channel in which the information was received and information not yet collected to determine an appropriate insurance policy product. In this way, a common application is utilized to provide a full suite of insurance policy products to customers while allowing the customers to submit relevant information over any number of different channels.

Various aspects also include determining whether a customer qualifies for temporary or permanent life insurance coverage based upon a customer's information and/or answers to one or more knockout questions, and if so, providing the customer with a binding receipt of temporary insurance coverage upon payment for a period of time in which the underwriting may be completed.

    Abstract: An insurance method and computer system provides incentives and insurance ratings to a policy holder. The policy holder may opt to participate in an incentives program in which the method and system may receive baseline personal health data that indicates a baseline health condition and a target goal to improve or maintain that baseline health condition over a specified time period. The method and system may then receive and analyze personal health data generated by a sensor that monitors a current health condition over the specified time period to determine whether the target goal has been achieved. The method and system may provide an indication of an incentive to the policy holder in response to receiving the personal health data generated by the sensor, and may provide an indication of a further incentive in response to determining that the target goal has been met.
