Abstract: A method for a healthcare and wellness web platform includes storing a health information document of a user on a web platform in response to a health information request. The stored health information document is controllable by the user. The method includes representing a plurality of healthcare providers as members of a healthcare team on the web platform. The healthcare team may be specific to the user. The method includes granting access to the health information document to the plurality of members of the healthcare team according to configurable criteria.
