    Abstract: An implement, particularly useful as a medical implement for applying a suture to a body tissue, includes a handle and shaft formed with a pair of channels extending from the proximal end of the handle through the distal end of the shaft and through a pair of jaws carried at the distal end of the shaft. The pair of channels converge towards each other to a cross-over point, and then diverging away from each other where existing at the distal end of the shaft. The channels are dimensioned to enable a piercing instrument to be passed first through one of the channels to pierce the body with a first bore, and then through the other of the channels to pierce the body with the second bore, such that the two bores have entry ends precisely spaced a predetermined distance from each other, and exit ends precisely spaced a larger predetermined distance from each other.

The cutting guide has a reference block for location against the bone relative to the edge of the bone at one side and at least one arm which is shaped so that it can extend from the cutting guide over the face of the head. The arm can be located positively with respect to features on the head so that the location of the reference block along the axis of the bone can be set with reference to features on the head of the bone which are contacted by means of the arm.

The surgical instrument further includes, at its distal end, a socket dimensioned to receive an anchor to be implanted in a bore in the bone, which anchor also has secured thereto a suture to be passed through the pierced soft tissue and to be tied to the bone. The invention is particularly described with respect to a method wherein the tissue to be attached is labrum tissue detached from the glenoid bone in a shoulder joint.

The metaphyseal component can initially include a convex articulating surface that can be replaced with a concave surface during the shoulder arthroplasty procedure or in a subsequent revision surgery. The metaphyseal component can also include a feature to facilitate removal of the component from the bone once it has been disengaged from the diaphyseal component.
