    Abstract: A sampling kit form for storage and shipping of organic material. The form includes first and second coextensively joined plies, with adhesive and release coating placed at select locations therebetween such that the form surfaces are substantially free of lateral or longitudinal undulations, which allows the forms to be stacked without leaning or sagging. The first ply also includes a die cut that circumscribes a removable inset portion. Upon removal of the inset portion, adhesive disposed on an inner surface of the second ply is exposed, thus permitting the organic material, such as hair samples, to be placed on the exposed adhesive coating. Fold lines in the sheet establish preferential folding locations so that the organic material samples can be encased between layers of adhesive and release coating within the form once it is folded. The form also includes a postcard that can be detached along a perforated line of weakness.

    Abstract: This need is met by the present invention wherein an improved shipment form is provided comprising a partially releasable liner ply, a peel-out package label portion, and a peel-out signature tab. In accordance with one embodiment of the present invention, a shipment form is provided comprising a face sheet, a liner ply, and an adhesive interposed between the face sheet and the liner ply. The face sheet includes printed indicia for indicating an attempted delivery. The adhesive includes a laminating adhesive portion and a reusable adhesive portion. The liner ply defines a releasable liner ply portion adjoining a laminated liner ply portion. The releasable liner ply portion is configured to releasably adhere to the reusable adhesive portion while remaining adjoined to the laminated liner ply portion.

    Abstract: A linerless continuous series of pressure sensitive adhesive labels is provided which have on one or more selected portions thereof a series of imagable coatings which may be activated to form colored images or areas. The series of labels include a web having first and second surfaces, with a series of imagable coatings of color formers and color developers on the first surface of the web. A coating of a release material overlies the imagable coatings on the first surface of the substrate, and a coating of a pressure sensitive adhesive is positioned on the second surface of the web. The series of labels are wound or stacked so that the coating of pressure sensitive adhesive contacts the release material to permit individual labels to be dispensed.

    Abstract: A multipart form and label combination is provided which may be printed on both sides with variable and nonvariable information. The combination includes a face ply and a bottom ply, each of which include a pressure sensitive adhesive and a release agent coated on respective portions of their second surfaces. The face ply and bottom ply are adhered to one another such that a portion of the adhesive-coated surface of each ply is coextensive with the release agent-coated surface of the other ply. Each ply also includes a label portion which is removable from the combination. In a preferred embodiment, the multipart form and label combination is used as a packing list incorporating shipping and return labels.

The liner ply remains adhered to portions of the label when folded and applied to a surface to facilitate removal of various panels of the label.
