    Abstract: A computer-implemented method of facilitating communication between a care giver and a care provider. The method includes receiving identifications of one or more of primary complaints and selecting data collection questions based at least in part on the identified primary complaint(s). The questions are presented to the care giver in non-expert language and configured to help identify a medical problem associated with the medical issue, but not to diagnosis the medical problem. After responses to the questions are received from the care giver, an Intelligent Resident Note is constructed and displayed to the care provider. The care provider enters or selects a diagnosis and one or more medical protocols for inclusion in a management protocol. The management protocol may include one or more instructions related to treat the medical problem that are sent to the care giver. The care giver confirms when each instruction has been performed.
