Abstract: Provided are systems and methods to create patient log entries using open-ended electronic messaging (e.g. email, text messaging, IM). The systems and methods allow users to update a patient log using widely available communication devices (e.g. cell phones or internet) without the devices needing to have web access and/or to use tailor-made applications. The invention also allows users to update the patient log with limited or no upfront training and describes methods to share the patient data among users using various interfaces.
