    Abstract: A non-invasive diagnostic system assesses the optimal timing for aortic valve replacement (AVR) by quantifying left-ventricular Frank-Starling reserve before irreversible myocardial damage occurs. The system (i) acquires left-ventricular ejection time (LVET) and other systolic-time intervals from optical or vibrational sensors positioned on the patient, (ii) induces a reversible preload change—e.g., passive leg raise or posture transition—to create a controlled venous-return increment, (iii) processes the paired baseline and post-maneuver waveforms to extract systolic time interval metrics, and (iv) analyzes the ?LVET/?preload relationship against historical or population references. A diminished LVET response signals loss of contractile reserve, enabling timely AVR while myocardial changes remain reversible. The platform integrates measurement hardware, a data-processing engine, a data-analysis module, and a reporting interface, and may be configured as a wrist, ring, chest, or ear sensor.

    Abstract: An assessment of basal cardiac fitness uses noninvasively obtained physiological measures to determine the presence of a basal physiological state and cardiometric measures to determine cardiac fitness. In the presence of preload independence and cardiac vagal control, cardiometric signals are acquired that are sensitive to cardiac function and include information indicative of the opening and closing of the individual's aortic valve. The cardiometric signals are analyzed by a cardiac fitness assessment system for the determination of basal cardiac fitness. The invention assures preload independence through the assessment of observational physiological parameters or the use of physiological parameters obtained following alterations in venous return. The invention assures the presence of cardiac vagal control by assessment of measured parameters, including the presence of respiratory sinus arrhythmia.

The noninvasive method and apparatus use changes in transmural pressure to create detectable changes in peripheral venous vascular volume for the determination of central venous pressure. Transmural pressure changes can be manifested by intravascular or extravascular pressure changes. The system is noninvasive and uses optical measurements of venous volume in the presence of transmural pressure changes. The relationship between the transmural pressure change and the change in vascular venous volume is combined with anatomical measurements to determine the central venous pressure of the subject. Central venous pressure can be used to determine hemodynamic status of the subject to include fluid overload in the heart failure patient.

Embodiments obtain a noninvasive optical pulse plethysmogram from the systolic and diastolic phases of the cardiac cycle for subsequent morphologic waveform analysis to determine left ventricular outflow tract anomalies. Left ventricular outflow tract abnormalities alter the rate of increase in flow during early systole and decrease in flow during late systole. These flow variances at the aortic valve are amplified as the pulse wave moves to the periphery due to a combination of the ventricular-aortic interaction, pulse augmentation, and reflections at branching vessels and changes in diameter.
