    Abstract: A screw guide for connecting to a bone plate includes a body having proximal and distal ends spaced from each other along a direction. The distal end has a coupling mechanism for coupling to the bone plate at an orientation. The body defines at least one channel that extends along a central axis and is configured to receive a bone fastener therein in a pre-loaded position. The coupling mechanism is configured such that the central axis substantially aligns with a central axis of a fastener hole of the bone plate when coupled to the bone plate at the orientation. The body has a retention mechanism at least partially located within the at least one channel and configured to apply a retention force to the bone fastener for retaining the bone fastener within the channel in the pre-loaded position.

The cutting edge is designed for placement in contact with the skin adjacent to the percutaneous device so as to define a leading edge thereof in a primary anticipated direction of migration of the percutaneous device. Kits and assemblies that include sleeves and percutaneous devices are also disclosed. Further disclosed are methods of reducing tension in a patient's skin following placement of the percutaneous device via use of the sleeve, as well as methods of adjusting and readjusting the assembly upon placement.

    Abstract: A clutch mechanism connects a rotating shaft to an outer clutch member having a clutch keyway. The clutch mechanism permits the shaft to rotate in a normal direction and prevents the shaft from rotating in a reverse direction when the outer clutch member is rotationally fixed to the housing. A key is moveable into an engaged position with the clutch keyway to rotationally fix the outer clutch member to the housing. The key is also moveable into a disengaged position to permit the outer clutch member to rotate relative to the housing. Thus, the shaft is permitted to rotate in both the normal direction and the reverse direction. A positive, mechanical and/or visual indication of whether the shaft is capable of operating in both directions may also be included. The assembly may include a sealed interior space in common with the machine including the shaft.

The sidewall has at least one slot for allowing outward expansion of the sidewall against the through hole to thereby lock the bushing at a selected angle relative to the axis of the through hole. The fastening screw has a threaded shaft for insertion through the bore of the bushing and threads into bone to secure the bushing and fixation device to bone. The head of the fastening screw fits in the bushing and includes a radial wall and open end defining a recess. The radial side wall has at least one slit for allowing outward expansion of the radial wall thereby outwardly expanding the sidewall of the bushing.

    Abstract: A bone fixation assembly for securing a fixation device such as a bone plate, to bone includes a fixation device including a through hole; a bushing having a sidewall with a bore and a slot; a fastening screw with a threaded shaft and a head including a recess; and a locking screw with a body capable of being threaded into the head of the fastening screw to expand its radial wall. In use, the bushing is seated within the through hole of the fixation device at a selected angle and the fastening screw is inserted through the bore of the bushing such that its head fits in the bushing and its shaft is threaded into bone. The locking screw may then be threaded into the head of the fastening screw to expand the radial wall of its head resulting in the locking of the fastening screw, and bushing at a selected orientation within the through hole.
