Abstract: The present application is generally directed to systems for monitoring, updating, and facilitating care of a patient. The system may have a database storing medical data for the patient, a platform communicatively coupled to the database, and a user interface accessible by one or more users. The platform may have an access control module for authenticating user credentials and for controlling read and write access for users by providing a plurality of assignable user access levels. The user interface may have a social page for facilitating communication between users, a medical page for accessing medical data for the patient, and a care team page for accessing data related to a care team, which may include one or more users. In some embodiments, each of the one or more users may have an assigned access level for each of the social page, medical page, and care team page.
