    Abstract: A uniplanar pedicle fastener that is bottom-loaded and has a two-part pivoting mechanism is provided. The bottom loading is effected by particular complementary shapes of the fastener head and an opening in the bottom of the housing. The pivoting mechanism is provided by a protrusion and recess in the fastener head and housing bottom, respectively. The complementary dimensions of the fastener head, protrusion, housing opening, and recess permit both the bottom loading at one orientation, and, at a second generally perpendicular orientation, the pivoting articulation. The uniplanar pedicle fastener of the present invention can be attached to other pedicle fastener assemblies as part of a spinal stabilization apparatus.

    Abstract: A joint spacer therapeutically maintains separation of bones of a joint. A carriage is slideably retained within the frame and has at least one ramped surface. An actuator screw is threadably engaged with the frame, and rotatably connected to the carriage, to cause the carriage to slideably move within the frame when the actuator screw is rotated. First and second endplates engage the bones of the joint, and each has at least one ramped surface that is mateable with the ramped surface of the carriage, whereby when the carriage is slideably moved by rotation of the actuator screw, the endplates ramped surface slides against the carriage ramped surface to cause the endplates to move along an axis transverse to the longitudinal axis of the frame, to increase the height of the spacer. Piercing elements are connected to the carriage to pierce bone of the joint when the carriage is moved.

The clamps can be used in addition to or to replace hooks that grasp onto bone members, such as the lamina. One example of such a clamp is an in-line clamp that includes a central opening for receiving a rod member, a first opening for receiving a set screw and a second opening for receiving an elastic member therethrough. Another example of such a clamp is an off-set clamp that includes an upper plate, a bottom plate, and an opening for receiving a rod therein. The upper plate can be separated from the bottom plate to make space for an elastic member that can be secured within the plates. Tulip clamps that utilize one or more elastic members are also provided.
