    Abstract: Provided among other things is a sampling system for determining an amount or type of contamination a narrow, elongated passageway in a medical device, said sampling system comprising: (a) a fluid supply system that supplies to said passageway a sampling liquid for flowing through said passageway and a gas for flowing through said passageway; and (b) a receiving container that receives liquid from said passageway, wherein said sampling liquid is sterile, wherein said sampling liquid is configured to allow recovery of viable pathogens from the passageway, and wherein said sampling liquid comprises an amount and selection of surfactant effective to enhance the dislodgement of Enterococcus faecalis and Pseudomonas aeruginosa bacteria from the narrow passageways.

    Abstract: A cartridge is provided for dialysis or other blood processing therapy. In the cartridge, fibers may be substantially uniformly distributed near a midplane, but near an end, in the inter fiber space, there may be void flow channels, which may cause fluid flow in the inter fiber space to transition within a short region to uniform flow with minimal stagnation zones. Void flow channels may be be radially oriented, introducing fluid from the outer circumference, or axially oriented, introducing fluid along the axial direction through passageways through the potting material. The fluid flow in the inter fiber space may be perpendicular to the fibers, or radial with respect to a cartridge longitudinal axis. The cartridge may have blood flow in the inter fiber space, and flow of dialysate or ultrafiltrate in the lumens of the fibers, or the opposite situation.

    Abstract: Embodiments of the invention pertain to cartridges, systems and methods for performing hemodialysis and related extracorporeal blood treatment modalities and therapies, in which blood flows in the inter fiber space and dialysate flows in the lumens of hollow fibers. Appropriate connectors and fitting orientations may be provided. There may be provided orbital distributors, fanning of fibers, and features to promote uniformity of fiber spacing in the fiber bundle. Orbital distributors may contain contoured surfaces, flow redirectors, non-uniform-conductance flow elements, through-wall distributors, and other features. There may be subdivision of the fiber bundle into two groups of fibers with separate control fluid to each group. Appropriate systems may be provided for various therapies. Flow past the fibers may be parallel, transverse or other configuration.

    Abstract: A tympanostomy tube containing a polymeric material and a drug is described. The polymeric material may comprise a non-resorbable polymer and a water-soluble polymer. The drug may be released when the device is contacted by an aqueous liquid such as middle ear exudate during otitis media. The implant may contain sufficient drug to treat multiple episodes of otitis media or to catch subsequent episodes in their early stages as a prophylactic. The device may be hydrophilic and further may contain a surfactant. The implant may be porous. The implant may have enhanced surface area at places exposed to middle ear liquid and may have enlarged mass so as to contain an increased total amount of drug. Intentional reorientation of the patient's head may aid in moving drug-containing liquid around the middle ear cavity. Similar implants can be used in other mucosal cavities.

    Abstract: In embodiments of the invention, there is provided a dialyzer or filter comprising hollow fibers, in which blood flows on the exterior of the hollow fibers, and dialysate or filtrate may flow on the inside. The external surfaces of the hollow fibers may have properties of smoothness and hemocompatibility. The fiber bundle may have appropriate packing fraction and may have wavy fibers. Optimum shear rates and blood velocities are identified. Geometric features of the cartridge, such as pertaining to flow distribution of the blood, may be different for different ends of the cartridge. Air bleed and emboli traps may be provided. Lengthened service life may be achieved by combinations of these features, which may permit additional therapies and applications or better economics.

The base reinforced fibers are pre-coated with a tailorable tri-layer coating, such as Ti--TiN--Ti. Preferably the TiN layer is graded so as to have metal-rich outer surfaces, such as titanium-rich TiN, providing excellent bonding affinity for the base titanium layer, bonded to the surface of the fibers, such as silicon carbide, and for the outer titanium layer, bonded to the titanium aluminum matrix, and a compound core, such as stoichiometric TiN, providing a stable interfacial barrier against chemical reactions, whereby the tensile strength and resistance to cracking of the composite is preserved even at elevated temperatures of 900.degree. C. or higher.
