Longitudinal flanges depend transversely from longitudinal sides and diverge. Lateral ends depend inwardly and have mating surfaces on inside surfaces of the lateral ends for mating substantially flush with the edge surfaces which support the lateral ends and flanges. Lateral flanges depend transversely from lateral ends and diverge. The concrete insert includes a nut box insert comprising a bottom wall mounted in the housing for sealing an open bottom of housing and for sealing the gaps between the edge surfaces and the mating surfaces. The housing and the nut box insert forming a nut box, with a nut slidably mounted in the nut box, the nut box for constraining rotation of the nut for the threaded hole in the nut to receive a threaded fastener through an access slot in housing.
