    Abstract: A patient wristband form has a transparent ply with an upper surface and a lower surface, and a pressure sensitive adhesive coating on the lower surface. A release ply has an upper surface and a lower surface, with a release coating on the upper surface. The transparent ply is removably mounted on the upper surface of the release ply by the pressure sensitive adhesive coating. A die cut in the transparent ply defines an elongated wristband. An opaque coating is provided on the upper surface of the transparent ply in a central portion of the elongated wristband. A die cut in the transparent ply defines an overlaminate label that is sized to cover at least a part of the central portion of the elongated wristband, thereby covering indicia printed on the opaque coating. A paper ply may also be mounted on the release ply by pressure sensitive adhesive. The paper ply defines a plurality of labels that may be printed at the same time as the wristband.

    Abstract: An information band for mounting an electrically conductive circuit, such as a radio-frequency identification (RFID) tag. The band comprises a base strip and a top strip. A single sheet of label stock can include a plurality of such bands such that the sheet can be passed through a conventional printer to accept printed indicia onto a surface of one or more bands. These bands can be die cut to facilitate individual removal. The top strip can be either integral with the base strip or a separate piece that can be affixed to a surface of the base strip. Once indicia has been printed to the band, the RFID tag may be manually affixed between the base strip and the top strip, where adhesive disposed therebetween secures the RFID tag. The adhesive may be peripherally deposited such that an adhesive-free zone is formed between the two layers so that when the RFID tag is attached, it does not come in contact with the adhesive.

    Abstract: A tuck label for placing on a shipping package. The tuck label includes a substrate portion and a single sheet label portion with adhesive included on a surface. The label portion further includes one or more fold lines that define upper and lower adjacent print panels, both of which can receive printable indicia. A score line is placed in the label portion to define a mount region, a printable surface region and a pull tab region. The pull tab region is disposed between the mount region and the printable surface region, and includes a pull tab with at least one layer of adhesive so that, upon folding of the label portion along the one or more fold lines, the pull tab becomes multi-layered for increased strength and durability.

    Abstract: A multipart form and label combination is provided which may be printed on both sides with variable and nonvariable information. The combination includes a face ply and a bottom ply, each of which include a pressure sensitive adhesive and a release agent coated on respective portions of their second surfaces. The face ply and bottom ply are adhered to one another such that a portion of the adhesive-coated surface of each ply is coextensive with the release agent-coated surface of the other ply. Each ply also includes a label portion which is removable from the combination. In a preferred embodiment, the multipart form and label combination is used as a packing list incorporating shipping and return labels.

    Abstract: A tuck label construction which is simple and inexpensive to manufacture, is capable of being printed by a wide variety of conventional automated printing devices, and which can be printed with automated control systems such as bar codes is provided. In one form, the label includes a face ply having first and second major surfaces and a liner ply having first and second major surfaces. The first surface of the face ply includes an adhesive thereon which is adhered to the first surface of the liner ply, and the first surface of the liner ply contains a release agent thereon. Each of the face and liner plies includes a pair of end panels having center panels therebetween, with the end panels on the face ply being defined by lines of weakness between the end panels and the center panel on the face ply, and the end panels on the liner ply being Separated from the center panel on the liner ply.

    Abstract: A construction for a laminated card or label is provided including a transparent film having a pressure sensitive adhesive on one surface thereof and liner stock having first and second major surfaces. The first surface has a release coating adhered to the pressure sensitive adhesive on the film. Variable information may be printed on the second surface of the liner stock within the die cut area, and the die cut card removed from the transparent film and replaced printed side down onto the adhesive surface of the film, thereby laminating the card to the film. The card is then peeled away from remainder of the construction at the die cut around the film and folded with the indicia facing outward to laminate the sides the card together. The printed surfaces of the card are direct contact with the adhesive, thus preventing tampering of the card.

    Abstract: A business form such as a label, tag or the like is provided in which a thermally imagable substrate has on selected portions of its surface a plurality of different colored areas. At least one of the colored areas is obscured by thermally imaging the substrate in a selected area so as to provide color coded information. The colored areas may be preprinted as half-tone screened areas on the substrate in a number of different patterns and arrangements. The form may be printed with both color coded information and variable information in a single printing pass.
