The one or more clients are individuals receiving treatment or assistance. Each of the one or more clients are assigned to an account. Goals are established for each of the one or more clients. Data associated with each of the one or more clients is compiled as received from one or more monitoring devices. A determination is made where the goals are being met in response to the thresholds for the compiled data. Alerts are automatically communicated in response to the compiled data varying from a threshold to become significant for one of the one or more clients.
