    Abstract: Disclosed are an apparatus, system, and method for endotracheal tube placement that allow for ventilation of the patient throughout the process of placing an endotracheal tube. The apparatus includes a T-shaped adaptor having a first lumen for coaxial connection to an artificial airway and a second lumen that intersects the first lumen for connection to a ventilator. A cap is provided over the distal end of the first lumen, the cap having an opening that may be closed with a flexible plug, blocking air from exiting through the first lumen when the second lumen is attached to a ventilator. The opening is sized to receive a fiber optic bronchoscope and to flexibly seal against the exterior of the fiber optic bronchoscope as it passes through the cap.
