    Abstract: Systems and methods for removal of thrombus from a blood vessel in a body of a patient are disclosed herein. The method can include: providing a thrombus extraction device including a proximal self-expanding member formed of a unitary fenestrated structure, a distal substantially cylindrical portion formed of a net-like filament mesh structure, and an inner shaft member connected to a distal end of the net-like filament mesh structure; advancing a catheter constraining the thrombus extraction device through a vascular thrombus, deploying the thrombus extraction; retracting the thrombus extraction device to separate a portion of the thrombus from the vessel wall and to capture the portion of the thrombus within the net-like filament mesh structure; and withdrawing the thrombus extraction device from the body to remove thrombus from the patient.

The printable area is accessible through the void and is configured to receive indicia from a printer through the void while the wristband is releasably secured to the second portion. Upon removal of the wristband from the second portion, the second panel is foldable over the first panel to laminate the printable area.

    Abstract: A patient-identification wristband. The wristband comprises a strap. A terminal portion of the wristband has an upper section and a lower lamination section. The upper section comprises a printable ink defining a print area, a first slot inboard the print area and between the print area and the strap, a second slot outboard the print area, and a third slot outboard the second slot. The lower lamination section has a fourth slot below the second slot and a fifth slot outboard the fourth slot. A size of the fourth slot is bigger than a size of the first slot. A length of the upper section is greater than a length of the lower lamination section such that when the lower lamination section is folded over for lamination, the lower lamination section covers only a first part of the print area and leaves a second part of the print area exposed.

The first and second adhesive strips respectively extend along a top edge and a bottom edge of the shipping label. The printable area is between the first and second adhesive strips. The liner associated with the shipping label is configured to be removably secured to the first and second adhesive strips. The printable area is adapted to be printed while the liner is removably secured to the first and second adhesive strips.

    Abstract: A combination wristband and label form. The form includes a polyester section having a single-ply wristband defined by die cuts therein. The wristband includes a foldable portion having a first panel and a second panel. An outer surface of one of the first panel and the second panel is coated with a coating configured for receiving printed indicia. An outer surface of at least the other of the first panel and the second panel has adhesive to allow for the first panel to be adhesively secured to the second panel after the coating has received the printed indicia. The void exposes the coating on the outer surface of one of the first panel and the second panel to allow the printed indicia to be printed on the coating through the void. Upon folding the foldable portion, the printed indicia is sandwiched between the first panel and the second panel.

A strap extends laterally from the first panel in a first direction and a tab extends from the second panel in a second direction opposite to the first direction. The tab is configured to be adhesively adhered to the strap to secure the single-ply wristband around a person's appendage. The single-ply wristband is of unitary construction and is devoid of paper.
