    Abstract: Decision support services relating to palliative care are provided including identifying patients who potentially would benefit from a palliative care program, providing palliative care to a population of patients, and managing care of the population. Patients may be identified using a combination of patient-centric data and evidence-based logic to determine patients who likely qualify for palliative care. The identified patients may be stratified or subdivided into one or more categories, so that corresponding palliative care programs may be identified and patient care may be prioritized. These identified patients may then be matched with suitable health care providers. Additional palliative care services are described for generating palliative care workflows, patient worklists, managing patient transitions, referrals, and assignments.

    Abstract: Methods, computer systems, and computer storage media are provided for utilizing a healthcare facility's antibiogram information to generate customized antibiograms. One or more user-selected filters are applied to the information in the antibiogram data store. Filters include organism type, antimicrobial type, date range, encounter type, healthcare facility unit, and end-user role. Using the selected filters, a customized antibiogram is generated and presented on a user interface.
