Abstract: A method for generating a treatment plan for a patient history information is provided to a physician in electronic form. Patient examination information is provided in electronic form. Patient diagnostic test results is provided in electronic form. A plurality of candidate diagnoses is automatically selected using the patient history information, patient examination information and patient diagnostic test information. A plurality of treatment plans is presented to the physician from a predefined list of treatment plans, wherein each of the plurality of treatment plans is comprised of at least one of a medical test, a medical procedure, a prescription, a pre-certification, a school excuse or a work excuse, and wherein each of the plurality of treatment plans corresponds to at least one of the plurality of candidate diagnoses. A treatment plan is selected from the plurality of treatment plans. At least a portion the selected treatment plan, is executed.

Abstract: A HIPAA compliant for patient check-in at an office of a health care provider using a identification card. The card includes a substrate that carries a computer-generated representation of a name of the patient, a name of a medication, a medical condition, treatment, test or procedure, a place and date for when the medical condition was diagnosed, medication was prescribed and medical treatment, test or procedure was performed, a name of a health care provider diagnosing the medical condition, prescribing the medication and performing medical treatment, test or procedure and a result of the medical test. The card also carries an encoded, computer-readable identification number that identifies the patient in a medical information database. The card may also carry a computer-readable code that stores an encoded representation of the human-readable information. An identification card that carries information about a pet.

Abstract: A method and associated apparatus for generating a patient record that is compliant with insurance guidelines. The method comprising providing a medical transcription system that includes a plurality of fields that may be filled with either a default value or a non-default value. A template is provided for a doctor to view the default values for each one of the plurality of fields, the default values for each one of the plurality of fields may vary based on an exam level. The non-default values is applied to each one of the plurality of fields where the default value is inappropriate. The non-default values are input into the medical transcription system, and the medical transcription system inputs the default values. The compliant patient record is generated using the medical transcription system. In one aspect, fields in letters can be filled using information applied as the default and/or non-default values.
