Abstract: Systems, apparatuses, methods, and computer program products are disclosed for managing server-based patient centric medical data (PCMD). An example method includes transmitting a first template to a first computing device and transmitting a second template to a second computing device. The example method further includes receiving first patient data provided in response to the first template and receiving second patient data provided in response to the second template. The example method further includes generating a first record based on the first template, the first patient data, the second template, and the second patient data. The example method further includes generating a second record based on the second template and the second patient data.
