    Abstract: Methods and devices comprised of a collapsible insert member and shaping member for treating obstructive sleep apnea are described. The insert member is placed in the tongue or other tissue forming the airway of the patient and has a lumen to accommodate the shaping member. The shaping member is inserted into the insert member by the patient just prior to sleep and removed upon waking. A variety of shaping members are available to treat patients on an individual basis, i.e., to match their particular anatomic condition that is causing their apnea or snoring. The shaping member can be made from nitinol, stainless steel, or any other known biocompatible implant material capable of providing a change in the tissue response to stress.

A sliding tube assembly has a sliding tube in sliding arrangement with the inner cavity of the casing for moving in translation in an axial direction relative to the casing. A pair of traveling nuts is connected to the sliding tube assembly for moving with the sliding tube in the axial direction, the traveling nuts being operatively engaged to the threaded shaft for converting a rotational motion of the threaded shaft into a translation of the sliding tube, the traveling nuts being on opposite sides of a member of the sliding tube assembly connecting the traveling nuts to the sliding tube.

    Abstract: A device is disclosed for the securement of dermal tissues. The device provides approximation and eversion of the tissue as well as the placement of a fixation element that bridges a wound. The fixation element may be produced in several fixed or dynamic configurations that may or may not alter the ability to engage tissue in response to stresses placed upon the fixation member post-deployment. The delivery device inserts the fastener percutaneously through the epidermis into the sub-dermal position without entering the wound margins with any part of the approximation portion of the delivery device.

There is a step-wise change in thickness at a location between the first location and the second location. Having greater thickness at the periphery of the dome-shaped member may improve stiffness of the diaphragm and may allow for an increased break-up frequency. Having thinner material elsewhere in the dome-shaped member may allow the mass of the diaphragm to be kept low and may result in better acoustic sensitivity.

The fluid transfer assembly is selectively engageable by the patient for transferring fluid between the two chambers for modifying the rigidity, flexibility, and/or shape of the flexible chamber with minimal or no change to the volume of the implant.

The device includes a retracting element and a tissue engaging element that promotes tissue ingrowth around or onto the retracting element. The device may be implanted through the oral cavity alone or by using a trocar or a hand-held delivery system to deliver the device through the pharyngeal wall. Alternatively, the device may be implanted using an open, direct visualization approach from the side of a patient's neck.
