The protective guard is adapted for longitudinal movement between a first position substantially enclosing the obturator blade and a second position to at least partially expose the obturator blade.

    Abstract: Systems, kits, and methods for establishing percutaneous access are described. A system typically includes a deformable variable diameter cannula sleeve (DVDCS) and a guidewire. The DVDCS will generally have two states, an initial, un-activated condition and a second, activated condition, brought about and controlled under the discretion of the operator. The methods comprise creating an initial access point and tissue tract with a needle, positioning a guidewire through the initial tissue tract, passing the deformable variable diameter cannula sleeve over the guidewire through the tissue tract to a target blood vessel or cavity, and activating the DVDCS to effect radial deformation of the surrounding tissue. Use of the DVDCS reduces the risk of injuring tissue surrounding the tissue tract by lessening the axial forces imparted to the tissue. Kits comprise at least one deformable variable diameter cannula sleeve together with instructions for use.

    Abstract: A surgical access assembly including a housing, an elongate access member extending from the housing, an instrument seal disposed within the housing and a zero closure valve mounted relative to the housing. The closure valve includes a valve periphery and first and second lips depending from the valve periphery. Each lip includes a lip edge surface in juxtaposed relation to define a slit when the valve is in a closed position thereof. Each lip has a general arcuate internal lip surface leading to the lip edge surface to define a sloping configuration dimensioned to facilitate passage of a surgical instrument through the valve. Each of the lips includes a support rib depending from the internal lip surface, the support ribs defining a gap therebetween to be in spaced relation when the valve is in the closed position and being positioned to contact the surgical instrument during insertion through the lips.

    Abstract: Described herein are apparatus for performing lithotripsy. The apparatus comprise a CT scanner and a shock wave tube for generating shock wave pulses for fragmenting kidney stones. The CT scanner includes a guidance system which is utilized to guide and control the position of the shock wave tube relative to a patient. The shock tube moves along an track which may be coupled to the CT scanner or which may be movable relative to the scanner. The track may be circular or semi-circular. The shock tube is movable about three orthogonal axes, which movement is controlled by the guidance system. A method for performing the lithotripsy using the apparatus is also described herein.
