The sticker face and the adhesive are registrably positioned with respect to a fold line. This allows the adhesive to be transferred to the sticker face on demand by an easy, desirably manual process that involves folding the form along the fold line. The device comprises a base substrate, a layer of repositionable adhesive disposed on the base substrate, a layer of permanent adhesive adjacent to the repositionable layer opposite the base substrate, and structure providing a release surface. The base substrate and the structure providing the release surface can be moved apart so as to leave both of the adhesive layers on the base substrate with the permanent layer exposed, thereby enabling the selected substrate to be adhered to the exposed permanent layer. The label and the adhesive web over the label define the actual decal. In a preferred embodiment, the label is made from retroreflective sheeting. Labels printed with fixed information are provided on a master roll. The endmost label is printed with variable information, separated from the master roll, and then assembled into the decal assembly. For use, the releasable web is peeled back to reveal the adhesive web, the label, and the backing web.
