    Abstract: A surgical instrument for use with a robotic surgical system is described. The instrument includes an elongate shaft having a proximal end and a distal end. An electrically live wrist member is disposed at the distal end of the shaft. An electrocautery end effector is mounted to the wrist member. An interface is disposed at the proximal end of the shaft. The interface is removably connectable to the robotic surgical system. A conductor extends from the interface to the end effector so as to deliver electrical energy to tissue engaged by the end effector. A tip cover is disposed over the wrist member so that electrical current can only be conducted to tissues through the exposed end effector (e.g., to promote blood coagulation during usage) and not to other parts of the patient's body.

The graft attachment assembly includes an attachment member including a base portion having a convex top surface and a branch portion having a passageway therethrough. The branch portion projects outwardly from the base portion. A clamp member having a concave bottom surface is configured to sealingly engage the top surface of the base portion and an opening is dimensioned to slidably receive the branch portion. The clamp member is slidable about the branch portion to a position adjacent the base portion to clamp tissue therebetween. A locking member is slidable about the branch portion and dimensioned to secure a vessel thereabout.

Electrical isolation can be achieved using an insulating main tube through which drive cables extend from a backend mechanism to the effector, an insulating end cover that leaves only the desired portions of the effector exposed, and one or more seals to prevent electrically conductive liquid from entering the main tube. Component count and cost may be further reduced using a pair of pulleys that are shared by four drive cables.

    Abstract: An apparatus includes two clamp assemblies on opposing sides of an operating table. A cross bar is attached to the assemblies and extends across a lowered leg section of the table. A pivot joint is coupled to the cross bar, a first linkage couples the pivot joint to a first ball joint, a second linkage couples the first ball joint to a second ball joint, and a medical device is coupled to the second ball joint through an adapter. Each joint has a spring mechanism resisting its movement and a hydraulic cylinder to allow its movement. Depression of a foot pedal actuates all hydraulic cylinders so that an operator may position the medical device by moving the pivot joint and attached first linkage rotationally about and directionally along the cross bar, the second linkage rotationally about the first ball joint, and the medical device rotationally about the second ball joint.

    Abstract: This application is directed to a surgical stapling device for performing circular anastomoses. The surgical stapling device includes a handle portion, an elongated body portion and a head portion including an anvil assembly and a shell assembly. The handle portion includes a rotatable approximation knob for approximating the anvil and shell assemblies and a firing trigger for actuating a firing mechanism for ejecting staples positioned within the shell assembly. The firing trigger forms one link of a two bar linkage provided to actuate the firing mechanism. The anvil assembly includes a tiltable anvil which will tilt automatically after firing of the device and unapproximating the anvil and shell assemblies. The head portion also includes a retractable trocar assembly which is slidably positioned within an anvil retainer and is automatically advanced and retracted upon attachment and detachment of the anvil assembly onto the anvil retainer.

    Abstract: An aortic punch for creating an aortotomy in a wall of a luminal structure. The aortic punch includes a housing having distal and proximal ends, first and second plungers, a first return spring and a cutting assembly. The first plunger is movable relative to the housing to expose a barb from the distal end of the housing for piercing and catching the wall of the luminal structure. The first return spring biases the barb proximally toward the distal end of the housing such that the barb pulls the wall of the luminal structure into contact with the cutting assembly. The present disclosure also relates to a method of forming an aortotomy in a luminal structure.
