This approach permits a suture attachment which lies entirely beneath the cortical bone surface, and which further permit the attachment of suture to the bone anchor without the necessity for tying knots, which is particularly arduous and technically demanding in the case of arthroscopic procedures.

    Abstract: A suturing instrument and methods for placing mattress stitches in soft tissues comprises an elongate shaft with a stationary jaw and a movable jaw disposed at a distal end thereof. The movable jaw is coupled to a handle grip at the proximal end of the shaft and is configured to manipulate the movable jaw into open and closed positions respective to the stationary jaw. The jaws are configured to permit atraumatic grasping of soft tissues to be sutured. The stationary jaw is comprised of a serrated face, incorporating apertures through which needles may be driven distally into and through the grasped tissue and into needle capture cans attached to opposing ends of a single strand of suture material. The serrated upper jaw is configured with capture means adapted to accept and capture the needles and suture. The handle is released to open the movable jaw, after which the instrument may be withdrawn, trailing the suture, and leaving a mattress stitch in the grasped tissue.

    Abstract: Implant and delivery systems for soft tissue repair which affix soft tissue portions to a region of bone are described. Generally, the assembly includes two bone anchors preloaded through an inserter handle such that each anchor is penetrated into the bone directly. The first anchor is inserted through the tissue and into the bone, where it is locked into position. The first anchor has a suture or wire that protrudes through the tissue and threads through the second anchor and is secured to a rotatable suture reel located along the handle. The second anchor is inserted through the tissue and into the bone independently of the first anchor. Once the second anchor is deployed, the suture or wire is tensioned to secure the soft tissue to the bone and a suture plug within the second anchor is deployed to lock the suture or wire in place.

    Abstract: A knotless bone anchor and method for securing soft tissue, such as tendons, to bone, includes a plurality of discrete components. In one variation of the invention, the bone anchor includes a proximal toggle component, an intermediate plug component, and a distal sleeve component. Suture is looped around the plug component and once the anchor is actuated, the suture is compressed between the plug and the sleeve components. Advantageously, one or more of the components may be made of a biocompatible polymer. The polymeric components may be detachably joined to a drive shaft using a plurality of sacrificial fills. The sacrificial fills are broken or severed during deployment of the anchor. Related instruments and methods are also described.

Thus, failure rates are greatly decreased over conventional techniques, and the inventive procedures are significantly easier to perform than conventional techniques.

    Abstract: A suture loop is formed in a hollow braided suture by feeding one end of a length of suture through a part in the braid of the suture and into the inner lumen formed by the hollow braid. The braided configuration of the suture allows it to be expanded in diameter by pushing and reduced in diameter by pulling. Said end of suture is passed continuously through said inner lumen forming a loop of suture with a single tail. The loop may be tightened by pulling on said first end of the suture while pushing on said outer hollow braid. The loop may be locked by extending or pulling on said outer hollow braid to reduce its diameter and lock it down around said first end of the suture.

The methods include providing a biodegradable radiodense implant and taking a tissue sample from a biopsy site within a breast of a patient. The biodegradable implant is then positioned at the biopsy site. The tissue sample is tested and the biopsy site is then relocated. In one embodiment, the entire implant is radiodense. In another embodiment, the entire implant is biodegradable. Methods of using a biodegradable implant having a radiodense material and a biodegradable implant that is visible using an imaging system are also included.

    Abstract: A knotless suture lock and bone anchor wherein a suture, looped through a tissue, is threaded through a plurality of body holes in the anchor to cinch the suture and tissue to the anchor without tying a suture knot on the tissue, and wherein the anchor is adapted for embedding in a bone. The tissue is secured to the anchor by suturing the tissue without a suture knot, fastening the standing leg portion of the suture on a suture leg-anchoring structure, and threading the working leg portion of the suture through the body holes. Pulling on the working leg portion of the suture tightens the suture on the body structure and cinches the loop without a suture knot on the tissue. The bone-embedding structure comprises a plurality of barbs adapted to resist pullout of the anchor from the bone.

The suture attachment to the bone anchor involves the looping of a length of suture around a pulley within the bone anchor, tightening the suture and attached soft tissue, and compressing the suture against the bone anchor. The bone anchor may be a tubular body having a lumen with a locking plug that compresses the suture therein. The pulley may be a pin located near a distal end of the tubular body around which the length of suture is looped. Alternatively, a pulley may be a bridge portion of the tubular body between two spaced apertures in the wall of the body. The locking plug may include a shaft and an enlarged head that interferes with the tubular body to provide a positive stop.
