Abstract: A system for providing health care documentation is disclosed. The system includes one or more of a local location and a remote location including a plurality of devices including one or more patient examination note-taking devices, and one or more of patient diagnostic devices, wherein the plurality of devices are utilized to create and collaboratively modify a clinical document component file of an electronic medical document of a patient; a communication platform connected to the plurality of devices, wherein the communication platform permits communication between the one or more local location and the remote location; and one or more databases that are disposed at the one or more local location and remote location, wherein the one or more databases stores the electronic medical document of the patient.
