    Abstract: An apparatus (and corresponding method) is provided for assisting consumers in obtaining personalized medical information and services, such as health services for behavioral health disorders and learning difficulties, in accordance with a preferred embodiment of the invention. The apparatus in accordance with a preferred embodiment of the invention an initial assessment server and monitor server that combines patient medical and family history reporting with assessment, scoring and reporting, diagnosis support, treatment planning, outcome reporting, and other useful information in an integrated online system. Standardized input and data formats may be used to ease exchange of patient data between medical databases and resources. The system monitors, preferably using technology such as artificial intelligence, user inputs and data exchange to constantly improve performance. With the system, referrals can easily be made to other professionals involved in treatment.
