    Abstract: An ablation apparatus has an expandable member that is inserted into an organ of a body and ablates all or a selected portion of the inner layer of the organ. Electrolytic solution fills the expandable member, and the expandable member includes a plurality of apertures from which electrolytic solution flows from the expandable member. First and second fluid conduits, which can be first and second conforming members, are in a surrounding relationship to the expandable member. The second conforming member, including a conductive surface, is made of a material that provides substantial conformity between the conductive surface and a shape of the inner layer of the organ. A plurality of electrodes is positioned between the two conforming members. The expandable member serves as an insulator to RF energy. Each electrode includes an insulator formed on a surface of the electrode positioned adjacent to the second conforming member.

    Abstract: A medical probe apparatus comprising a catheter having a stylet guide housing with at least one stylet port in a side thereof and stylet guide means for directing a flexible stylet outward through at least one stylet port and through intervening tissue to targeted tissues. The stylet guide has an integrated circuit or semiconductor apparatus at the distal end thereof to generate electromagnetic radiation directly at the point of the desired target tissue. The stylet includes at least one semiconductor or integrated circuit radiation emitter adapted for electromagnetic radiation when electrically energized. An elongated electrical insulator includes proximal and distal ends adapted to be introduced into a body opening, such as the urethra. First and second elongated electrical conductors are electrically isolated by the insulator.

    Abstract: A medical probe device including a catheter having a stylet guide housing with one or more stylet ports in a side wall thereof and a for directing a flexible stylet outward through the stylet port and through intervening tissue at a preselected, adjustable angle to a target tissue. The total catheter assembly includes a stylet guide lumen communicating with the stylet port and a stylet positioned in said stylet guide lumen for longitudinal movement from the port through intervening tissue to a target tissue. The stylet can be an electrical conductor enclosed within a non-conductive layer, the electrical conductor being a radio frequency electrode. During the ablation operation, the tissue immediately adjacent the stylet can become desiccated, thereby necessitating an increase of current flow and a loss of ablation efficiency.

    Abstract: A transurethral needle ablation device for the treatment of the prostate of a human male using radio frequency energy wall comprising a sheath having a lumen extending therethrough. A guide tube assembly is slidably mounted in the lumen in the sheath and having a lumen extending therethrough. A needle electrode is slidably mounted in the lumen in the guide tube assembly. An insulating sheath is disposed about the needle electrode so that the distal extremity of the needle electrode is exposed. A handle adapted to be gripped by the human hand is mounted on the proximal extremity of the sheath. Levers are carried by the handle for causing bending of the distal extremity of the guide tube assembly at an angle with respect to its longitudinal axis whereby the lumen in the guide tube assembly can be directed so that it faces the urethral wall.

    Abstract: According to the present invention, an anastomosis catheter with internally mounted cam pieces holding a plurality of curved needles adjacent apertures in the side of the catheter is provided. The curved needles are attached to lengths of suture material which are installed to run along the outer surface of the central cam pieces to their distal end, where they reverse back along the inner core of the catheter. When the central cam pieces are withdrawn, and moved proximally, the cam surface forces the curved needles out the associated apertures. As the catheter is deployed in a hollow organ, such as a human urethra, the curved needles, as they are deployed, grasp the end of the urethra, which then can be held in position for suturing to the bladder. The other end of the suture materials could have straight needles attached to them to facilitate attaching the urethra to the bladder.

    Abstract: A medical probe apparatus comprising a catheter having a stylet guide housing with one or more stylet ports therein and stylet guides for directing one or more flexible stylets outward through respective stylet ports and through intervening tissue to target tissue. The active stylets emit radio frequency RF energy which establishes an RF current in the patient. The current, being densest at the stylet tips, causes heating of the surrounding tissue to initiate ablation of the target tissue. One aspect of the invention includes temperature sensing stylets at the location of the RF emitting stylets to monitor the temperature of the tissue being ablated. This allows the surgeon to have increased knowledge about the abating action under way in order to control the intensity of the applied RF current, or to adjust the amount of time the current is applied, or both.

    Abstract: A method and an apparatus is disclosed for delivering controlled heat to perform ablation to treat the benign prosthetic hypertrophy or hyperplasia (BPH). According to the method and the apparatus, the energy is transferred directly into the tissue mass which is to be treated in such a manner as to provide tissue ablation without damage to surrounding tissues. Automatic shut-off occurs when any one of a number of surrounding areas to include the urethra or surrounding mass or the adjacent organs exceed predetermined safe temperature limits. The constant application of the radio frequency energy over a maintained determined time provides a safe procedure which avoids electrosurgical and other invasive operations while providing fast relief to BPH with a short recovery time. The procedure may be accomplished in a doctor's office without the need for hospitalization or surgery.

    Abstract: A tissue ablation apparatus includes a delivery catheter with distal and proximal ends. A handle is attached to the proximal end of the delivery catheter. At least partially positioned in the delivery catheter is an electrode deployment device. The electrode deployment device includes a plurality of retractable electrodes. Each electrode has a non-deployed state when it is positioned in the delivery catheter. Additionally, each electrode has a distended deployed state when it is advanced out of the delivery catheter distal end. The deployed electrodes define an ablation volume. Each deployed electrode has a first section with a first radius of curvature. The first section is located near the distal end of the delivery catheter. A second section of the deployed electrode extends beyond the first section, and has a second radius of curvature, or a substantially linear geometry.

    Abstract: A medical probe for the treatment by radio frequency ablation of a target volume in tissue of a human having a urethral channel extending into the base of a bladder along a longitudinal axis. The probe comprises an elongate probe member having proximal and distal extremities and is sized so that it can be introduced into the urethra. The elongate probe member has a passage extending from the proximal extremity to the distal extremity. Guide cannulae are mounted in the passage of the elongate probe member and have proximal and distal extremities with the distal extremities of the guide cannulae being in the vicinity of the distal extremity of the elongate probe member. Each of the guide cannulae have a lumen extending therethrough from the proximal extremity to the distal extremity. A radio frequency conductive electrode is disposed in each lumen. An insulating sleeve is coaxially disposed on each radio frequency electrode. A control handle is secured to the proximal extremity of the elongate probe member.

    Abstract: An RF treatment apparatus provides multi-modality treatment for tumors and other desired tissue masses, and includes an RF indifferent electrode and an RF active electrode. The RF indifferent electrode has a compacted, non-deployed state, and an expanded deployed state. In the deployed state, the RF indifferent electrode forms a helical structure with four or less coils that surround an exterior of the tumor. The indifferent electrode becomes a microwave antenna when it is coupled to a microwave source. RF ablation energy is delivered to the tumor by the RF active electrode after it has been introduced into the tumor. Both electrodes are coupled to an RF energy source. The indifferent electrode is coupled to a microwave source. Either or both of the electrodes is hollow and include fluid distribution ports for the purpose of providing a chemotherapeutic agent to the tumor site. Ablation energy is supplied to the tumor by the active electrode.

    Abstract: An ablation apparatus has a balloon that is inserted into an organ of a body and ablates all or a selected portion of the inner layer of the organ. Electrolytic solution fills the balloon, and the balloon includes a plurality of apertures from which electrolytic solution flows from the balloon. The flow rate of electrolytic solution is dependent on the pressure applied to the balloon by the electrolytic solution. A conforming member, with a conductive surface and a back side, is made of a material that substantially conforms to a shape of the inner layer of the organ and delivers the electrolytic solution and RF energy through the conductive surface to the inner layer. Advantageously, difficult to access areas are reached with the inclusion of the conforming member. Optionally positioned between the conforming member and the balloon is a porous membrane. A printed circuit is printed in or on the conforming member and delivers RF energy to selected sections of the inner layer.

    Abstract: A medical ablation device comprising a flexible RF electrode wire or tube, the terminal portion thereof extending through an insulating sleeve axially moveable thereon. The electrode and surrounding sleeve extend through a portion of a rigid tube having a distal end adapted to be inserted into the body of a patient. Tabs are attached to the proximal end of the sleeve and electrode for advancing them together from the terminal end of the tube, for retracting the sleeve from the distal end of the electrode to expose a predetermined area of electrode surface, and for retracting them together into the terminal end of the tube. The electrode can also be a hollow tube, the proximal end thereof being adapted to be connected to a suction source for aspiration of tissue adjacent its distal end or it can optionally contain a fiber optic, the end thereof being enclosed within the tube and closely adjacent its distal end.

The energy is independently transferred directly into the respective tissue around the stylets in such a manner as to provide respective tissue ablation without damage to surrounding tissues. Automatic shut-off of both channels occurs when any one of a number of surrounding areas to include the urethra or surrounding mass or the adjacent organs exceeds predetermined safe temperature limits. Automatic shut off of the individual channels occurs when the temperature of the tissue proximal to the respective stylet exceeds respective predetermined values.

At least one measuring electrode or ablation device is attached to the distal end of the tube. The outer sheath and the inner tube are rotatable relative to each other so that the electrode or device can be rotated within the heart without rotating the outer sheath. The sheath is also axially movable relative to the tube to selectively enclose or expose the electrode or device for use.

    Abstract: A method of this invention for treating body tissues containing cancerous cells or non-malignant tumors with RF ablation, alone or in combination with systemic or localized chemotherapy comprising introducing a stylet comprising an electrode surface and a sleeve longitudinally moveable thereon into the vicinity of the body tissues, retracting the sleeve from a portion of the electrode surface, and supplying RF power to the electrode surface sufficient to heat the tissue to a temperature of above about 45.degree. C. for a time to cause reduction of tissue mass in the vicinity of the electrode. The RF power supplied to the electrode surface is sufficient to effect a desiccated fluid diffusion barrier capsule surrounding the body tissue being treated. The stylet can include a hollow tube having fluid distribution ports therein, and the method can include the step of passing fluid through one or more distribution ports into the body tissue being treated.
