Abstract: Methods and systems are provided for assigning an individual to a stratum associated with a risk of generating a high level of health care-related costs. An electronic device receives information on the diagnosis of a medical condition for the individual. The device then identifies a gap in the individual's medical care for the diagnosed medical condition and associates the gap with an indexed value related to the severity of the gap in care. The device then assigns the individual to one of a plurality of strata based on a health care profile of the individual, where the health care profile includes the indexed value related to the severity of the gap in care.
