The system and method include the steps of providing a plurality of questions relating to a patient, where each question has at least two selectable answers, and wherein each selectable answer has associated therewith a corresponding score, receiving one of the selectable answers for each of the plurality of questions, calculating a total score corresponding to the sum score of each of the answers selected, and generating a post-acute care referral if the total score meets a predetermined threshold value.
