    Abstract: The present invention enables the creation of a structured, outcomes-based problem-specific patient care plan that is automatically updated every visit. As clinicians perform initial and ongoing assessments, NDoc continuously applies rules-based logic to identify new patient problems and determine which existing problems have been resolved. Interventions and outcomes relating to active problems are highlighted dynamically, drawing clinicians' attention to current patient issues while at the same time providing a complete clinical charting environment for comprehensive documentation. A multi-session software system is also disclosed, which allows users to concurrently perform several disparate tasks without having to complete and close one process before starting another.
