Abstract: A system for managing patient care provider referral. The system includes a database which includes records for a plurality of patient care providers. The system also includes a processor, and a memory storing software executable by the processor. The software includes a lookup tool configured to retrieve patient care provider records from the database, and a transaction processing tool configured to enable a referring patient care provider to select a referred patient care provider for a patient based on the retrieved records. The system can further include an electronic health record (EHR) tracker configured to track updates to an EHR of the patient, as well as a survey tool configured to collect feedback from the patient regarding the referred patient care provider. Methods associated with the system and the various tools are also disclosed.
