FIG. 5 is a right side view thereof;

FIG. 6 is a top plan view thereof; and,

FIG. 7 is bottom plan view thereof.

In the drawings, the broken lines illustrate portions of the combined screen and screen shroud of a portable cognitive assessment device, that forms no part of the claimed design.
