The vertically-oriented internal solid lines represent cut lines and the evenly sized dashed lines represent fold lines in FIGS. 1, 2, 4 and 5. The broken lines consisting of a long dash followed by a short dash showing of the eyelet in FIGS. 1-6 are for illustrative purpose only and form no part of the claimed design. The thickness of the blank is that of a conventional paperboard.
